Overview:
A mission-driven and growing hospice provider is seeking a compassionate, experienced Home Health Registered Nurse to join its Warrenville-based team. This is a full-time, field-based role offering flexible scheduling, strong leadership support, and the opportunity to make a lasting impact on patients and families during their end-of-life journey.

Base salary: $80,000–$90,000 annually

Position Highlights:
Branch Census: 12–15 patients and growing
Leadership: Reports to an Executive Director (RN) who remains involved in patient care
On-Call: Shared with Clinical Manager and Executive Director (every other week); triage nurses take first calls
Territory: Primarily Will and DuPage Counties, with coverage into parts of Cook County
Key Responsibilities:
Conduct physical and psychosocial assessments and develop individualized care plans
Manage pain and symptoms with evidence-based interventions
Provide education and emotional support to patients and families
Collaborate with interdisciplinary team members for coordinated care
Accurately document all assessments, care plans, and follow-up in the EHR (Netsmart)
Participate in on-call rotation and quality improvement efforts
Qualifications:
Active RN license in Illinois
1–2+ years of hospice nursing experience required
Strong assessment, documentation, and communication skills
Committed to providing compassionate care—this is more than just a 9–5
Team-oriented and able to cover the full territory as needed

Resource 1 was founded out of something deeply personal—the exceptional home health and hospice care my grandfather received during the final chapter of his life. His experience left a lasting impression on our entire family and became the inspiration behind everything we do.
My grandfather grew up a farm boy in rural West Texas, never venturing beyond the county lines until age 19. His life soon took a remarkable path: he served courageously as a bombardier in World War II and later as a jet fighter pilot during the Korean War. After his military service, he returned to his roots—his cherished farm—where he found peace in the familiar rhythms of the land he loved.
In his later years, it was compassionate home health and hospice professionals who made it possible for him to remain at home, surrounded by comfort, dignity, and family. Their care made all the difference.
